Book Purchase Request Form

This form is to request that the library make a one-time purchase for a book, DVD, or CD. Please check the library catalog to verify that the library does not already own the item before submitting a recommendation for purchase. - Thank you.

To request purchase of a dvd, please complete the course reserve request form instead. Please note that currently new dvds are only purchased for required class viewing and all dvd requests will be placed on reserve.
